Use this form to sign up for the Men's Fellowship Golf Tournament (August 15, 2024).
You may include up to three additional golfers to complete a team. If less than three golfers are added, the tournament committee will add additional players to form a complete team. Individual golfers only need to complete the top portion and make a payment to register.
Keep in mind, a player is not officially registered until they pay the $125 entry fee (per person) on the next screen.
